People who are Homeless, at Risk of Homelessness and Under Housed

People who are homeless and registered as clients at Bramalea CHC can access primary health care from an integrated team of health care professionals that includes: nurse practitioners, family physicians, registered nurses, community health educators, medical office assistants, social workers, community dietitians, physiotherapists, and chiropodist.

Other services include:

  • Counselling to address a wide range of mental health and substance use issues
  • Support accessing other community based supports such as subsidized housing, shelters and food banks

In addition to offering individual supports to individuals who are homeless, we also work on larger initiatives and partnerships to end social conditions that contribute to homelessness. We are connected with the Region of Peel Street Outreach Team to ensure they refer homeless and under-housed people to our primary care team as well as our other programs and services.
